- This is like the cover page of the league. If you want the important details, this is your doc. Print some copies for your youth group or friends.

| Rule Sheet |
- Brush up on the rules of the game. I don't mind answering questions or giving clarification, but please look this over before you play. That way we can get right to it on Tuesdays.

| Roster Form |
- Print and fill out as a team. This can be mailed or handed into the St. John's church office, with your team fee of $75 by June 1st. The earlier you turn this in the better, and don't forget the consent forms!
- Checks can be made out to St. John's Lutheran Church and either mailed or dropped off at:
St. John's Ev. Lutheran Church, 809 South 6th Avenue, West Bend WI, 53095.

- Very Important! Each participant of every team needs to fill this out in order to play. It would be best to hand this in with your teams roster, but if that's not posible I will accept them prior to your game. Particpants that are 18+ years of age still need to fill this out and sign it.
